Lost: Season 2 Episode 9 - the scene where Kate after taking out Sawyer outside sees the horse again, yes that is a hand of an animal trainer which we were not supposed to see. Unfortunately cameraman spoiled the scene for us, and now we know that there is a crew on the island too, so many things on the island huh… ;) click on the image to see a larger view.

That shot does not exist on the episode. I looked through the whole scene… nothing. I have no doubt that that is a doctored photo. The closest photo I could find was /Users/Steveo/Desktop/Snapshot 2006-05-07 16-24-58.tiff
If you are watching DVD version it might have been removed already, try to watch the TV version.
